All new Member Associations seeking to register their association teams with USA Hockey and CAHA for play during the regular season (September 1-February 28) must submit their application and supporting documents to the CAHA Membership Chair by November 30 of the prior year.
All membership applications for associations that seek to begin operation in the 2024-2025 season will be submitted online. The portal will not save your information if you close the application before you reach the end.
These documents provide the information and supporting documents needed to help you prepare before you begin your application.
The application will not be brought to the CAHA Board of Directors for a vote on membership unless the application is complete and all supporting documents are received and reviewed to be in good order by the CAHA Membership Committee.

Electronic versions are required in .pdf, .doc, .docx, .xls, .xlsx or .csv format. Applicants will have the opportunity to upload documents when completing the online application.
Bylaw 10 - Member Associations defines the new Member Association application process, documents required and timelines.
All applicants must complete the CAHA Member Association Agreement and upload with their application.
Page 1 and 2 must be initialed in the space provided at the bottom of the form.
Page 3 must be signed in the space provided on the form.
All youth associations (members under the age of 18) must submit a completed and signed SafeSport Agreement with their application.

All Member Associations, with the exceptions of new associations approved for the 2024-2025 season, must complete the Member Association renewal prior to August 1, 2024. Note: the 2024-2025 season of the USA Hockey Registry to claim players, coaches and managers and to create team rosters will not be released to your association until your association's renewal is complete.
This season associations must provide association information and a completed, signed CAHA Member Association Agreement and USA Hockey SafeSport Agreement.

All documents uploaded must be a full page render of the original document. Documents sized at less than 8 1/2" X 11" will not be accepted.
